Hi,
First download, install and run Malwarebytes for Mobiles. If it finds malware, delete the malware and then see how your phone behaves.

If the problem has returned then check if you have any downloaded apps which have 'permission' to access the camera? Notice in the image, under 'hardware controls' that it says ''take pictures and videos''
Go to Menu > Settings > apps > downloaded > tap on all the apps and scroll down to view 'permissions' to see which ones have access to the camera.
-If it happens regularly enough try 'force' stopping an app with this 'permission' and see if you can find out which one is causing the problem.
