I would first check for any liquid damage on the charging port dock connector on the logic board.
 
You can try cleaning the headphone jack with tweezers or something equivalent as there may be dust or lint stuck deep inside the headphone jack.
 
If there are no signs of liquid damage on that connector then the headphone jack is most likely damaged from liquid and the whole charging dock assembly would need to be replaced.
 
 
The charging dock consists of the main microphone used for calls, the headphone jack, connection to the loudspeaker and the main antenna to get cellular / service provider signal for the sim card / phone.
