Why is the sound in my left headphone louder than my right?

I use my headphones everyday and in any outlet I possibly can, as time progressed I have begun to lose sound in my right headphone. What can be the issue here?

This could be two issues, either your device is set to Mono, or your adapter may be damaged. I would like to remind you that these headphones are Apple Product friendly, so use in other devices may leave you with less than perfect results.
