From what I could see there is a bolt on the top, one on the front, and one one the outside just behind the bumper.

Hi,
Here are some images from the 2004 Nissan Titan service repair manual, that shows how to remove the 4 bolts that hold the headlamp and also how to adjust the headlamp.

there is 2 adjusting bolts for the lens angle. than normally 2-3 to hold the lens in place.
