replacement lense blurry video intermittent sound

I recently replaced the lense in my Silver Hero 3 due to a scratch on the original lense. But now when I take videos it is very fuzzy and the sound is intermittent. Just wondering what I might have done wrong in the replacement process to cause these issues?
