This sounds like a sender problem, you can replace the fuel pump that has the sender for fuel level integrated — another potential fix is to put in a couple bottles of fuel system cleaner on a half tank or under of gas. Often it will free the sender. But to replace everything you have to drop the tank. We had a Caravan with similar problem and just tracked miles very carefully.

How do u know if everything has to be replaced
No i always keep it full actually it's just a 1/4 used only go to work and surrounding area.
@Balerie Stokes if the fuel sending u it goes you have no choice. It all comes together as one unit. Fuel pump, sending unit, etc. They're typically one piece so if one goes, you have to replace the entire unit.

Suspect either a faulty gauge or sending unit in tank . Either way start by checking the wires on the fuel tank sending unit which also powers the fuel pump. Your looking for loose and or corroded connections.
