Square in middle of screen

There is a blue square in the middle of the screen that covers a good majority of the picture. Can you tell me what causes that and how can that be fixed

If I recall correctly, this is what happens when closed captioning is turned on when there is no CC signal. Try turning off closed captioning from the remote.
