cutting off while driving

1992 v6 toyota 4 runner cuts off while driving. replaced fuel filter,trottle sensor,checked fuel pressure. never had any problems with it , let it sat 2 ,3 months and noticed the cap was off of the dash port.

air/fuel ratio issue, vac leak, timing, is there any other symptoms that may have appeared whilst this was happening like lower MPG or issues starting the engine or louder than usual engine noise?
