Here are some links which should help. The first one shows how to dis-assemble the phone, which may be beneficial in showing how to remove the digitizer and LCD flex cable. The next two show how to remove the digitizer itself (although the second one not so much as it already removed but it does show the flex cable connection which the first one seems to gloss over)
