Is the fan vent clear and unobstructed?
-It may be that the fan is dusty and the vent is being obstructed preventing the removal of heat from the CPU which causes the CPU to run hot and therefore the fan keeps running, most probably at high speed, hence the noise.

It could also be that the CPU is running hot due to the thermal paste being caked and dried out, preventing a good transfer of heat to the heat sink assembly.
