Sim Card Stuck in phone
Card fell out of tray when putting it into phone and is inside the phone not on the tray
how do i get it out?

You’d have to open the phone and physically move the sim out through the holes in the sim tray using tweezers or anything with a fine point to avoid bending the pins.

Card is not in the holder. It slid off when i was putting it back in the phone.
It's like @strongbow said.
The phone has to be opened so that you can gently push the card back out of the SIM card reader, while hopefully not damaging the reader itself.
Here's a video that shows how to dis-assemble the phone that may help.
If doing this yourself is too daunting, contact a reputable, professional, mobile phone repair service and ask for a quote.

If the cover to the tray came off you need to bring it to a cell phone repair place and they can take the back of the phone off and get the tray out immediately. Same thing happened to me it didn't cost me a thing to get it fixed at batteries Plus

try getting it out with a needle or pick. If it doesn't come try removing the back cover of the phone by prying it open and getting access to the motherboard where the sim card holder should and then you should be able to remove it if you couldn't do it by just using a toothpick or a needle
